Medications for Camp Hancock

Dear Parents,

We follow the same rules for administering medications at camp as we do for administering medications at school.

We take with us any medications from the LS Office that your student has there already.

Any additional medications that you want sent to camp must be sent to school in a ZIPLOC BAG with your child’s name clearly marked:

1. FOR PRESCRIPTION MEDICATIONS: They must be in the original bottle with the prescription label attached. If this is not possible, then written physician orders must be provided with the medication.

2. FOR NON-PRESCRIPTION/OVER THE COUNTER MEDICATIONS:

They must be in the original, labeled container.

FOR EACH MEDICATION, an “ADMINISTERING MEDICATIONS” form must be completed, signed, and sent with the medication. This form can be printed from the Camp Hancock link on the Fifth Grade Team Web Page or obtained from the LS Office.
